Please use this form to request courthouse visits, tax records, and specialty research. If you need surname research or newspaper/file lookups for births/marriages/deaths, please use the Surname Request Form and Lookup Request Form.

Print this page and complete the form. If it is a specialty search, we will reply with a fee quote. If it is a courthouse visit, please enclose a check for $15; if it's a tax record search, please enclose a check for $30. Checks should be payable to the S.C.H.S. Please mail to:
